I authorize permission to The Empty Cup Society to use the image of myself to include but not limited to display, distribution, publication, social media, website or other use of photographs, and/or video taken of myself.



I give permission for my image to be used in print, video and digital media. I agree these images be used by The Empty Cup Society for a variety of purposes and that these images may be used without notifying me.
